Forms: Award Nominations

2010 Award Nominations

by Mary Jo Smith

February 15, 2010

Please use the form below to nominate your very own, very deserving NeASFAA members for an award. Nominations for the prestigious awards are closed as of March 6th. However you may still use this form to nominate persons for fun awards and to let us know about children and grandchildren.

If you have the inside scoop on one of your NeASFAA colleagues, please nominate someone (or a few someones) for a fun award. We can not promise a $10,000 prize like those on America's Funniest Home Videos, but we certainly will offer to laugh along as if we were.

Babies, Babies, Babies, we all love babies. Please make sure to let us know about those new children and grandchildren from the last year. List the baby's name as the 'Nominee'. Child or Grandchild under the 'Award Type' and other details in the 'Reason' section. You will need to submit this form more than once if multiple children are involved.

When having creative thoughts keep in mind that the theme for the conference is EXTREME MAKEOVER NEASFAA.
